In order to expedite your visit, Please bring the following items. 

  • All current medications in original bottles.
  • Insurance card(s) and a Photo ID.
  • Bring recent imaging (Chest X-ray, CT Scans, etc).

 New Patients, in order to finalize your registration process, please arrive 30 minutes before your scheduled appointment time. 

Please check with your insurance carrier to determine if you will need a referral before your visit.
